 Author  H1N1 vaccines NOT MUCH LEFT!

   I have only 50 shots left (from my 200)

ALL THE SHOTS ARE ALREADY RESERVED SO WE CANNOT ACCEPT ANY NEW BOOKED PEOPLE. ALSO WE CANNOT ACCEPT PATIENTS WHO ARE NOT REGISTERED ON OUR ROSTER! ANY PATIENT WHO IS NOT ON OUR ROSTER WILL BE DELETED FROM THE BOOKING! THIS IS DUE TO THE PUBLIC HEALTH NOT PROVIDING US WITH ADEQUATE AMOUNTS OF VACCINE SO WE NEED TO RATION IT.

ONLY FOR HIGH RISK GROUP
===People under age 65 with chronic health conditions
(such as diabetes, asthma, cancer or a condition that affects the heart, liver, lung, or immune system)
===Pregnant women over 20 weeks
===Healthy children aged six months to less than five years
=== People living in remote and isolated settings or communities

=== Health care workers
=== Household contacts and care providers of infants under six months or persons who may not respond to vaccines
Note: Children from six months to nine years of age should receive the vaccine in two half-doses, given at least 21 days apart. Individuals 10 years of age and older should receive one dose of the vaccine.
H1N1 flu vaccine can be given at the same time as seasonal flu shot and other vaccines.

I find this shot hurts for about 2-3 days . Some patients have a mild fever and headache for about 2-3 days after the shot.

   We also have Zostavax, the new shingles (Zoster) vaccine from Merck for patients over 60 years ago. It is a single shot priced at $180.00.

We have a new dedicated freezer that stores the vaccine at below -15C and it must be given within 30 minutes of thawing.

Merck told me there is backorder of this vaccine. I have nine in stock in my office as of Nov. 2, 2009

We have just received our first shipment of flu vaccine. For seniors first. H1N1 flu shot stock will come in November.
People under 65 should get H1N1 shot first in November and then regular shot in December.

For Saturday and after 5 pm appointments, YOU MUST BE REGISTERED (or be willing to register) in our patient roster. We are always closed on Sundays.

   If you are Dr. Ray Stein's patient for cataract preop, do not need to confirm your appointment . Just show up at Dr. Pun's office on the day and time that Cathy gave you. Please bring your list of medications and valid OHIP card. You can even speed up your visit by emailing your medication list to chinesemusic@rogers.com

   Drs. Ian Pun and Christina Lo are a husband and wife team. We have a nice practice of patients and have built up to full capacity. For your best possible care and for consideration for our fellow patients, please limit your visit to one or two important issues.

We will try to resolve and treat your medical problems as best as we can, with western evidence based medicine.

Please have consideration when booking prime-time spots after 5 pm and on Saturdays. We know you are all busy but let some other patients have these spots too!

Please arrive at your appointment time. If you arrive earlier, you may even wait longer!

We apologize sometimes the wait may be long sometimes due to emergencies but it's usually averages 20 minutes or at most 2 hours.

PLEASE DO NOT ASK TO JUMP THE QUEUE! Please be patient and enjoy our large flat panel TV.

   If you want Email support: Because of the volume of emails from patients, I kindly ask for a registration Email Fee (1 year of email contact with Dr. Pun) per person $30 or $60 for family of 4 (2 parents and children only) because this is an uninsured medical service (i.e. OHIP does not pay me for this) I am sorry to impose this small fee due to the increasing costs in rent, upgrading and maintaining my medical and computer equipment followed by the increased volume and demands of patients. There are practically no other physicians that I know of that provide email support. I will provide one complimentary email request before full payment. This fee is part of a block fee that is legally recognized by the Ontario College of Physicians and Surgeons. This fee is includes e-mail service provided by both Dr. Pun and/or Dr. Lo.

   Dr. Pun and Dr. Lo will no longer act as guarantors (ie. sign) for new passport applications. You do not need a doctor to sign if you are renewing a passport. We will not sign for citizenship cards also. This is due to our privacy concerns.

   If you come to have your blood taken, you may have to fast (not eat). This is usually for cholesterol and glucose tests. More info is at Gamma Dynacare Labs

   We have had two cases of H1N1 swine flu confirmed by nasopharyngeal swab last week. Before making an appointment , please tell the secretary if you have any fever, cough , muscle ache and sore throat. Also, if you have been in contact with anyone (i.e. student) who attends Agincourt Collegiate as there are many students sick there. Anyone with flu symptoms must wear a mask in our office.

   Dr. Pun will see referrals for Ministry of Transportation driver's exams, Dr. Raymond Stein's cataract preop exams and other trusted sources.
Please e-mail ianpun@rogers.com for authorization.

We absolutely do not accept any patients of other existing family doctors, especially ones in the area.
Dr. Lo will not accept any new patients or do female exams (pap smears) of patients from other family doctors.
We speak English , Cantonese and some broken Mandarin.
We use googletranslate for some Chinese translation.

Disability forms $20 per page (must be our regular patient)

Driver's examination form only $40
Certify photocopied document $10 per page
Vaccine document translation $20
PSW forms/ education form $25
Sign passport form (NO LONGER A PROVIDED SERVICE)
photocopy lab result 25 cents per page

Minor surgery - mole removal $50 each
Minor surgery - mole removal on face $70 each
Wart treatment (cautery and/or cryofreeze and podofilm) $10 each wart per treatment (2-3 treatment needed for total $20-30)
Genital wart treatment (cautery and podofilm) $20 each
Skin tags $10 each
TENS treatment/simple acupuncture $10 each treatment

Minimum charge for uninsured patient $50 ; preop exam $80 ;drivers exam $80

Registration Email Fee (1 year of email contact with Dr. Pun) per person $30

Dr. Pun / Dr. Lo will inform you in advance if the service is covered under OHIP. Please ask the doctors if you have any questions.

We accept cash, cheque or Interac Email Money Transfer only. Receipts will be given if requested. Some services may be covered under private (or company) insurance plans. Ask your insurance broker or company. Receipt will be given upon request.

Some vaccines may be covered under your company insurance (usually up to 80%).

These fees are standard amounts according to the Ontario Medical Association and are non-negotiable.

Rotateq (oral diarrhea vaccine for baby under 6months) $60 x 3 = $180
Varicella chickenpox vaccine - $80
Typhoid vaccine for travel - $40 each
Dukoral vaccine for traveller's diarrhea - $80
Adacel (Tetanus, diptheria, pertussis) - $40

Hepatitis A vaccine $60 x 2 = $120
Hepatitis A vaccine for child (under 18 years old) $30 x 2 = $60
Hepatitis B vaccine $30 x 3 = $90
Combo hep A+B vaccine (Twinrix) $60 x 3 = $180

for Hepatitis A vaccine we use these brands: Havrix, Vaqta or Avaxim.
for Hepatitis B vaccine, we use Engerix or Recombivax.

NEW Gardisil cervical warts vaccine, prevents cervical cancer = $500

Cash or cheque only on receipt of vaccine. No debit or credit card.

   If you are applying for life/disability/critical illness insurance. Please tell your insurance company or broker my name, address and fax number (416 - 848 - 7788 ) There will be NO CHARGE to you for a doctor's report for applying for private insurance (unless specifically stated in the policy).

   Dr. Pun and Dr. Lo will only complete disability and EI forms only for our regular patients. If you have been seeing other physicians, you must be contact your regular physician for completion of these forms. The cost of these forms is not covered by insurance or OHIP. Generally it is $20 - $40 depending upon complexity.
